The Client Registry is used to accurately identify individuals registering at hospitals, community health centres and connected pharmacies across Newfoundland and Labrador. Its database contains demographic information collected by all provincial hospitals, community health services, long-term care facilities, connected community pharmacies and the Medical Care Plan (MCP).
It is essential within the health system that individuals seeking care, treatment, or service be accurately identified by all health care providers involved with the individual’s care. Incorrect demographic information can lead to incorrect health histories being retrieved or new health information being recorded in the wrong file.
Each time you register for a medical appointment, present to a pharmacy or register for an MCP number the following demographic information will be collected, confirmed, or updated by the registration clerk or health services provider:
- Full name
- Address
- Phone number
- Mother’s name
- Gender
- Date of birth
- Health care number (MCP or alternate number if from another province or military)

Progress/Status
The Client Registry is fully operational and is currently integrated with a number of eHealth systems and the Regional Health Authorities.
